Job Summary: As a Client Experience Manager, you hold a pivotal role in ensuring the highest standard of care and satisfaction for our clients. Your extensive experience in home care, client management, including a minimum of two years with one year in a supervisory capacity, equips you to assess, coordinate, and elevate client experiences. Your dedication to professional development and commitment to compassionate care, as well as your compliance with (preferred) CPR certification and background checks, set the foundation for success in this role.

Responsibilities:

  • Holistic Assessments: You conduct comprehensive assessments of prospective clients, delving into demographic details, personal preferences, medical considerations, and daily living functionality. This thorough evaluation empowers you to tailor care plans that address individual needs.
  • Collaborative Care Coordination: You serve as a liaison between clients and other healthcare professionals, ensuring seamless communication and collaboration to meet client goals and requirements.
  • Resourceful Support: Leveraging your expertise, you provide valuable resources and referrals for supplementary services based on client assessments, enriching the overall care experience.
  • Engaging Onboarding: Initiating a positive client journey, you orchestrate standard experience calls for new clients and conduct home visits, fostering strong relationships while adhering to local regulations and company policies.
  • Community Outreach: Collaborating with the Community Outreach team, you facilitate assessments, extending Greenstaff Homecare's reach and impact within the community.
  • Continuous Improvement: You contribute to the Quality Improvement program, supporting the refinement of services and reinforcing the organization's commitment to excellence.
  • Documenting Excellence: Your meticulous documentation in the CRM system ensures that critical client information is accurately recorded and readily accessible.
  • Compliance Champion: By collaborating with the Caregiver Experience Manager and staffing, you ensure alignment with client needs and any changes, while also assisting in compliance matters.
  • Emergency Support: Your dedication extends to offering backup care to clients requiring personal care in emergencies, showcasing your dedication to client well-being.

Qualifications:

  • A minimum of two years' home care experience, with one year in a supervisory capacity.
  • Current CPR certification and clear criminal background check.
  • Preferred experience as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
  • Bachelor's degree in a related field is advantageous.
  • Dedication to ongoing education and professional development.
  • Reliable transportation and valid auto insurance.
